Your soul is trapped somewhere between life and death, endlessly tormented by a relentless wraith. The demo shows the full Hospital map, you have to study every object and decide if something has changed or not by choosing one of two doors. Get it right and you move forward. Get it wrong and you start over from the beginning. Every time you fail, the game shows you what you missed, so you learn as you go. Anomalies and jumpscares are randomized each run, so every attempt feels different.
